There are a number of processes to complete in order to complete the setup of Eyes on the Baby, for your organisation, which need to be completed prior to the start of programme delivery. Use these links to jump to that section on this page or scroll down to read through it all.

Complete the following form to register your interest in implementing Eyes on the Baby. The Durham Infancy and Sleep Centre (DISC) team will review your submission and be in touch about next steps.

DISC will initiate discussions with the strategic lead in your organisation to help to define the scope of the training and rollout across your organisation. This initial scoping will help you to visualise how Eyes on the Baby can be integrated into the work you are already doing and how it can dovetail with the work of partners across your region.
Please view our Programme Development resources which describe how the Eyes on the Baby package was defined and piloted and to see some of the resulting research outputs.
An outline of costings for the training will be issued. This will be defined by the size of your organisation and the breadth of the implementation. We will work with you to ensure the costings are viable for both parties. On agreement of the costings, we can move on to the contractual stage.
A contract will be issued, detailing the full terms and conditions of the training programme, training package and the commitments from both parties with regards to implementation.
Completion and return of the contract will initiate billing and purchase of the Eyes on the Baby package.
On return of the signed contract an invoice will be raised by our Finance Service, for the agreed costings, to allow you to purchase the Eyes on the Baby package. This invoice should be paid using our standard terms, as outlined in your contract.

An implementation date will be agreed with all parties, which will allow training to commence and all materials and resources to be released. Please refer to our comprehensive guide to Implementation.
A Certificate of Commitment will be issued to show that your organisation has committed to complete the training and implement the principles of Eyes on the Baby in relation to that training.
Setup and implementation can now commence. Our Implementation resources include a set of supporting documents including templates for Timeline of Rollout, Project Planning, Quarterly Check-in Meetings and Newsletters.
