To book an appointment with the Athlete Health Performance @ Durham team please email ses.technician@durham.ac.uk.
While our team looks you into booking your preferred date and time you will receive a set of documents to complete including:
- Medical Screening Questionnaire
- Service Terms and Conditions
Payment of the services is to be received in advance of your appointment. An invoice will be sent to you via email. Your payment will secure your appointment date and time.
